Cardio-CT

Cardio-CT makes it possible to obtain diagnostic images of the coronary arteries with high accuracy and reliability without an invasive cardiac catheterisation in our Hamburg radiology practice.

The amount of contrast medium required is comparable to that of a cardiac catheter examination.

Compared to a cardiac catheter examination, the radiation dose is sometimes lower.

What is important for the patient to know before the Cardio-CT examination:

It is optimal for the examination if the pulse is 60/min or lower.
This reduces the necessary radiation dose. To support this, a so-called beta blocker is often given in the preparation. This is a drug that lowers the blood pressure and slows down the heart rate.
Laboratory results (TSH basal / creatinine) must be available for the examination. Metformin-containing medication must be discontinued at least 24 hours before the examination.

The following indications showed a clinical benefit of cardio-CT examination:

  • Suspicion of the presence of coronary artery disease (CAD) at low and intermediate clinical probability.
  • For follow-up of coronary artery bypass.
  • On the evaluation of cardiac space demands.
  • For planning before transcatheter aortic valve implantation (TAVI)

The examination is particularly suitable to exclude coronary heart disease.

Performance of the Cardio CT

Lime score

The images are taken without contrast medium. Here, the degree of coronary calcification is measured quantitatively for each vessel individually and in total. The extent to which the amount of calcification has a prognostic value on the course of coronary heart disease is determined.

CT angiography of the heart

Due to the movement of the heart, a CT device with a high spatial and temporal resolution must be used for the examination. This reduces the probability that image artifacts will be generated by a cardiac arrhythmia disorder. In order to produce lower heart rate and variability, a beta-blocker is usually administered in advance. The examination is carried out under ECG derivation. Iodine-containing contrast medium is applied to visualize the vessels. Now bottlenecks within the coronary vessels can be detected and measured. It can be differentiated whether the narrow spots are caused by calcified or non calcified plaques.

If a coronary heart disease (CHD) is diagnosed in CT, an invasive cardiac catheter examination is usually required afterwards for confirmation and therapy. In patients with a low to moderate likelihood of CHD, coronary CT is well suited to rule out CHD.
