You can print out the registration forms by clicking on the PDFs. Please ask your doctor to fill in these forms and fax them to us for your first appointment.

Registration first presentation
Dear Patient,
So that we can immediately concentrate on the essentials during your first presentation, we ask for your help. for your assistance:
- First of all, please ask your general practitioner or the doctor who referred you to us to fill in your registration form and fax it to us. Please fill in the registration form for your first appointment and fax it to us.
